What is Home Health Care?

Is Home Health covered by Medicare or insurance?

A clinical nurse will visit your home to complete a comprehensive assessment, review your medications, evaluate your environment, and finalize your care plan.

Your assigned clinical team begins visits according to your plan. We monitor your progress, communicate with your physician, and adjust the plan as your needs evolve.
